Find Appropriate form by title, then click on "Download File". You can fill out the form on line then Print, or vice versa.
Please go to our "CONTACT" page and send us a memo with your child's name, so that we attach the payment to your child. We will send out a confirmation as soon as we receive all information.
You may mail in your registration form, print out the form and email it at FAMEartscamp@yahoo.com , or drop the form off at the front office of Woodland Elementary Charter School.
